The Illinois Department of Human Services (IDHS) announced a security incident that left over 705,000 health records exposed online.

This Illinois government data breach happened because staff posted internal planning maps on a public website without setting the correct privacy controls.

IDHS discovered the problem on September 22, 2025. The exposed records included:

• Names and home addresses
• Case numbers and status reports
• Demographic information
• Medical assistance plan names

Approximately 32,401 Division of Rehabilitation Services customers had their data exposed from April 2021 through September 2025. Around 672,616 Medicaid and Medicare Savings Program recipients had records accessible from January 2022 through September 2025.

Federal HIPAA regulations require agencies to notify affected individuals within 60 days of discovering a breach. IDHS didn’t issue its public notice until January 2, 2026, a full 102 days after discovery.

How IDHS Responded

Within four days, IDHS changed privacy settings on all affected maps. The agency created a Secure Map Policy that bans uploading customer-level data to public mapping websites.

IDHS states it has no evidence of misuse but cannot rule out unauthorized access. The agency is notifying affected individuals as required by law.
